What we need

Enough to accept the referral

You do not need a complete packet to refer. Send what is in front of you, and we will collect the rest from the family and from your office once we have a signed release.

  • Patient age and primary diagnosis
  • The skilled nursing need you are documenting
  • Georgia Medicaid status, if known
  • A family contact name and phone number
  • Your name, practice and the best way to reach you
  • County of residence, so we can confirm we staff it well
Who is appropriate: Georgia Medicaid members under 21 with a documented skilled nursing need — ventilator dependence, tracheostomy, complex airway, seizure disorders, enteral feeding and other medically fragile conditions. See the full list.

What happens next

After your referral lands

The administrative weight of a GAPP case sits with us, not with your staff.

We contact the family

Within one business day. We explain GAPP, answer the cost question, and confirm they want to proceed before anything else moves.

We verify benefits

We confirm Medicaid eligibility and assemble the GAPP funding packet ourselves. Your office does not chase authorisations for us.

RN assessment in the home

One of our registered nurses assesses the child where the care will happen, and identifies what the home actually needs.

Plan of care back to you

You receive the proposed plan of care for signature, and ongoing clinical communication from the nurse who is in the home.
